insurance reimbursement FAQs
Carol Chamblin accepts Blue Cross/Blue Shield PPO plans as an in-network provider. Claims are submitted for you with the use of medical codes. With the license to practice as an Advanced Practice Nurse (APN) with a specialty in lactation-related care by certification as an International Board Certified Lactation Consultant (IBCLC), coverage is not limited to 1-3 visits.
Claims are submitted for appointments until problems are resolved and plan of care completed.

You can call the Elmhurst Pediatric office in Elmhurst, IL and schedule an appointment. The office phone number is 630-832-3100. In addition to my private practice, I work at this pediatric office. You don’t have to take your baby to these doctors to see me there.
As an in-network provider, I can submit the claims for the use of the Medela Symphony pump.
A prescription is not needed. I have these pumps in-stock for your use. Usually BCBSIL covers 1 personal pump (e.g. Medela Pump In Style MaxFlow, Spectra, Motif, etc.) and also 12 months of the Symphony pump use. The Procedure Code for the Medela Symphony hospital grade pump is E0604.

Prenatal breastfeeding classes are designed to provide moms with valuable information and practical skills to help them successfully breastfeed their newborns. My goal is to offer a supportive and educational environment for mothers to learn about the benefits of breastfeeding and how to navigate common challenges.
